Our Clinical Director reviews your screening and medical records from referral sources. The clinical team evaluates appropriateness for our programs and approves admission -- typically within 1 hour.
Once approved, our admissions team calls you back within 24 hours to schedule your intake appointment. We work around your availability and can even accommodate virtual intakes when needed.

Bring your insurance card (front and back) along with a valid government-issued photo ID for verification during check-in.
Any referral documents, previous treatment records, discharge summaries, and a list of current medications including dosages.
Names, phone numbers, and relationships for at least two emergency contacts. Also bring any legal documents related to your care (court orders, FMLA forms).
A notebook and pen, comfortable clothing, water bottle, and any personal comfort items. Meals are provided during Partial Care programming.
